How the team works
Listen and identify
The team begins with conversations in the community, identifying vulnerable persons, listening to caregivers, and understanding what support is most urgent.
Document with dignity
Each beneficiary profile is documented with care, so assistance is traceable, respectful, and connected to the actual needs on the ground.
Follow through
After support is delivered, the team keeps communication open and returns to check progress, gaps, and the next practical step.
Every name in the register represents a real person, a real story, and a follow-up plan the team can stand behind.
